چکیده

PURPOSE Ultrasound-guided fine needle aspirate cytology fails to diagnose many malignant thyroid nodules; consequently, patients may undergo diagnostic lobectomy. This study assessed whether textural analysis (TA) could noninvasively stratify thyroid nodules accurately using diffusion-weighted MRI (DW-MRI). METHODS This multi-institutional study examined 3T DW-MRI images obtained with spin echo echo planar imaging sequences. The training data set included 26 patients from Cambridge, United Kingdom, and the test data set included 18 thyroid cancer patients from Memorial Sloan Kettering Cancer Center (New York, New York, USA). Apparent diffusion coefficients (ADCs) were compared over regions of interest (ROIs) defined on thyroid nodules. TA, linear discriminant analysis (LDA), and feature reduction were performed using the 21 MaZda-generated texture parameters that best distinguished benign and malignant ROIs. RESULTS Training data set mean ADC values were significantly different for benign and malignant nodules (P = 0.02) with a sensitivity and specificity of 70% and 63%, respectively, and a receiver operator characteristic (ROC) area under the curve (AUC) of 0.73. The LDA model of the top 21 textural features correctly classified 89/94 DW-MRI ROIs with 92% sensitivity, 96% specificity, and an AUC of 0.97. This algorithm correctly classified 16/18 (89%) patients in the independently obtained test set of thyroid DW-MRI scans. CONCLUSION TA classifies thyroid nodules with high sensitivity and specificity on multi-institutional DW-MRI data sets. This method requires further validation in a larger prospective study. Dynamic Contrast Magnetic Resonance Imaging (DCE-MRI) and Diffusion Weighted MR Imaging (DWI) for Differentiation between Benign and Malignant Salivary Gland Tumors

Background: Salivary gland tumors form nearly 3% of head and neck tumors. Due to their large histological variety and vicinity to facial nerves, pre-operative diagnosis and differentiation of benign and malignant parotid tumors are a major challenge for radiologists. Objective: The majority of these tumors are benign; however, sometimes they tend to transform into a malignant form. Functional M...

The Utility of Diffusion Weighted Magnetic Resonance Imaging in Detection of the Origin of the Brain Solid Metastatic Tumors

Background: Early diagnosis of brain tumors has significant effect on the treatment process. Brain metastatic tumors are usually diagnosed following the neurological symptoms in patients or incidentally after Computerized Tomography (CT) scan and Magnetic Resonance Imaging (MRI) requests of the brain. Objectives: Implementation of a new method for being informed about the origin of brain tumors...

Gliomas account for almost 80% of primary malignant brain tumors in adults. Magnetic Resonance imaging (MRI) is still the gold standard for diagnosis of brain tumors and brain 99mTc-tetrofosmin Single Photon Emission Computed Tomography (99mTc-tetrofosmin-SPECT) has been established as a useful tool for their evaluation. Preoperative Factors Predictive of Posterolateral Extracapsular Extension After Radical Prostatectomy

PURPOSE In radical prostatectomy (RP) procedures, sparing the neurovascular bundles adjacent to the posterolateral aspect of the prostatic fascia has often been suggested as a possible risk factor for positive surgical margins. Here we aimed to quantify the probability of extracapsular extension (ECE) at the posterolateral side of the prostate to aid in nerve-sparing decision making. MATERIAL...

Central gland and peripheral zone prostate tumors have significantly different quantitative imaging signatures on 3 Tesla endorectal, in vivo T2-weighted MR imagery.

PURPOSE To identify and evaluate textural quantitative imaging signatures (QISes) for tumors occurring within the central gland (CG) and peripheral zone (PZ) of the prostate, respectively, as seen on in vivo 3 Tesla (T) endorectal T2-weighted (T2w) MRI. MATERIALS AND METHODS This study used 22 preoperative prostate MRI data sets (16 PZ, 6 CG) acquired from men with confirmed prostate cancer (...
